1. Data konten yang dihapus (dan link-nya),
2. Penjelasan bahwa menurutmu ini kesalahpahaman atau bukan pelanggaran,
3. Info kontakmu (nama lengkap, alamat, email, dan nomor HP),
4. Pernyataan bahwa kamu bersedia menyelesaikan ini sesuai hukum yang berlaku,
5. Tanda tanganmu.

Setelah kami terima tanggapanmu, kami akan memprosesnya sesuai prosedur dan bisa saja mengembalikan konten tersebut, kecuali ada proses hukum lanjutan dari pihak pelapor.
